Privacy Statement
Information Collection
We collect information from users at several different points on the Sites. Examples of requested information can include, but are not limited to:
* Business information, such as name and location of office or practice
* TIN and Member (Social Security) Numbers
* Claims Information
* Personally identifiable information, such as mother's maiden name or a pet's name, so that security measures can be activated to protect your individual use of the Sites
* Group and employee information for eligibility purposes
Information Use
We are the sole owner of the information collected on this site and will not sell, share, or rent this information to others in ways different from what is disclosed in this statement.
Registration
You may browse most of the Sites without any registration process. However, to use certain interactive functions, a user must first complete a registration form. During registration, a user is generally required to give business information. This information is used to contact the user when necessary and to create a secure password for future use.
Cookies
A cookie is a piece of data stored on the user's computer. Usage of a cookie is in no way linked to any personally identifiable information while on the Sites. Once the user closes their browser, the cookie simply terminates. The benefit in setting a cookie is that the user does not have to log in a password more than once, thereby saving time while on the Sites. Examples where cookies are used on the Sites include, but are not limited to:
* Cookies are used so that offices making multiple entries/searches do not have to re-key specific information
If your browser is set to not accept cookies, you will be unable to use interactive sections of the Sites. However, we have no access to or control over these cookies and would recommend that you review those sites' privacy policies.
Log Files
We use IP addresses to analyze trends, such as site usage, and administer the site. IP addresses are not linked to personally identifiable information.
Our Disclosure of Your Information
As previously mentioned in this policy, as a general proposition, we do not sell or rent any personally identifiable information about you to any third party.
Due to the existing regulatory environment, we cannot ensure that all of your private communications and other personally identifiable information will never be disclosed in ways not otherwise described in this Privacy Policy. By way of example (without limiting the foregoing), we may be forced to disclose information to the government or third parties under certain circumstances, or, however unlikely, third parties may unlawfully intercept or access transmissions or private communications. We can (and you authorize us to) disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate, in connection with an investigation of computer fraud, intellectual property infringements, or other activity that is illegal or may expose us to legal liability. Therefore, although we use the highest industry standard practices to protect your privacy, we do not promise, and you should not expect, that your personally identifiable information or private communications will remain private.
Control of Your Password
You should not disclose your password to any third parties nor share it with any third parties. If you lose control of your password, you may lose substantial control over your personally identifiable information and may be subject to legally binding actions taken on your behalf. If you suspect unauthorized use of your User ID and password, you should immediately take steps to both change your password and notify us of potentially fraudulent activity.
Reviewing and Changing Your Personally Identifiable Information (Password)
Following registration, you can review and change the information you submitted during registration including your password. You must promptly update your information if it changes. Once posted, you cannot change or remove any public postings made in message areas.
Links
The Sites contain links to other sites. Please be aware that we are not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave the Sites and to read the privacy statements of each and every web site that collects personally identifiable information. This privacy statement applies solely to information collected by us on the Sites.
Surveys and Contests
From time to time, the Sites may request information from users for surveys or contests. Participation in these surveys or contests is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include contact information (such as name and shipping address), and demographic information (such as ZIP code, age bracket). Contact information will be used to notify the winners and award prizes. Survey information will be used for purposes of monitoring or improving the use and satisfaction of the Sites.
Security
The Sites take every precaution to protect users' information. When users submit sensitive information via the web site, your information is protected both online and off-line.
For example, when users are asked to enter sensitive information (such as TIN or Member Number), that information is password protected and also encrypted with the best encryption software in the industry - 128-bit SSL. While on a secure page, the lock icon on the bottom of Web browsers such as Netscape Navigator and Microsoft Internet Explorer becomes locked, as opposed to unlocked, or open, when you are just "surfing."
While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information off-line. All of our users' information, including but not limited to the sensitive information mentioned in this document, is restricted in our offices. Only employees who need the information to perform a specific job (for example, a customer service representative) are granted access to personally identifiable information. Furthermore, ALL employees are kept up-to-date on our security and privacy practices. As new policies are added, our employees are notified and/or reminded about the importance we place on privacy, and what they can do to ensure our customers' information is protected. Finally, the servers on which we store personally identifiable information are kept in a secure environment, behind locked doors, protected by a security card system.
Site and Service Updates
From time to time, we may send the user site and service announcement updates. Members are able to unsubscribe to service announcements. We communicate with the user to provide requested services and in regards to issues relating to their account via email or phone.
Correcting/Updating Personal Information
If a user's personally identifiable information changes (such as your office location or TIN), or if a user no longer desires our service, we will endeavor to provide a way to correct, update or remove that user's personal data provided to us. This can usually be done on the member information page or by writing our Customer Service department.
Notification of Changes
If we back changes to our privacy policy, we will post those changes on our home page so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. If any any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify users. Users will have a choice as to whether or not we use their information in this different manner. We will use information in accordance with the privacy policy under which the information was collected.
